Future Student Checklist for Toddlers and Preschool Students 

All items must be received prior first day of school.
Children must arrive NO LATER THAN 9:30 AM DAILY 

1. Florida Health Medical Forms Immunization
2. Florida School Entry Health Exam
3. Copy of parent’s picture identifications and all up-to-date phone numbers to call (3 minimum)
4. Family Picture
5. Registration Packet
6. Parent Handbook Signature Page
7. Full Uniform (all students must be in full uniform on first day of school)
8. Extra clothing in a clear school bag
9. Bedding in a clear zip-lock bag labeled with your child’s name
10. Diapers/Cream
11. All payments made prior to start date:

• • Registration
  • Deposit
  • 1st Tuition Payment

12. Classroom Supplies and Disinfection Materials must be brought in first day of school
13. Allergy and/or any Medical-Trauma Informed Waivers Signed for Special Care

Future Student Checklist for VPK (Voluntary Prekindergarten Program-4 years old By September 1^st) 

All items must be received prior first day of school.
Children must arrive NO LATER THAN 9:00 AM DAILY

3. Copy of parent’s picture identifications and all up-to-date phone numbers to call (3 minimum)

5. VPK Certificate
6. Registration Packet
7. Parent Handbook Signature Page
8. Full Uniform (all students must be in full uniform on first day of school)
9. Extra clothing in a clear school bag
10. Bedding in a clear zip-lock bag labeled with your child’s name
11. All payments made prior to start date unless child if ONLY VPK and not wrap-around:

9. Student Records from Previous School/Institution (testing/report cards/records)
10. All payments made prior to start date:

  • Book Fees
  • Computer Fees
  • Headphones (can be purchased separately)
